Health and Medical Information for 2023-2024
The MGCS School Nurse is available to assist with medical issues as they arise during the school day. She administers all daily scheduled medications as well as dispenses other “as-needed medications” on a case-by-case basis. Additionally, the nurse creates individual health care plans (IHPs) for all students attending school with illnesses that could potentially impact the student’s learning while at school. This includes but is not limited to, students with Type 1 diabetes, epilepsy, severe food allergies, and asthma.
In order for medication to be administered to your student, please complete this Permission to Administer Medication form and return to the school nurse.
If your student has Asthma or Allergies that require a medical response, please complete this Asthma/Allergy Action Card and return to the nurse to ensure the school has proper protocols in place.
